Health professionals and caregivers can choose the information that is needed and present it to families at relevant times. Developed as a collaboration between BC Children's Hospital and Canuck Place Children's Hospice, it is comprised of evidence-based information to guide in decision making, along with quotes, ideas and personal stories from others who have been there.
